Benefits (Pre-Tax)


Medical Huber offers two types of plans: a premium plan which has a mix of deductibles and co-pays for in-network services along with a prescription drug benefit and a core plan which has a higher deductible and then pays at 80% for most services. The core plan has the same prescription drug benefit. (applies to in network providers)
Dental The dental plan is designed with emphasis on preventative care, with cleaning and X-rays paid at 100% twice a year, there is a small deductible and the calendar year max for the plan is $1500. Orthodontia is covered at 50% with a lifetime max of $1500. Other services are covered at various rates, basic restorative care (fillings, root canals, etc) at 80%, major restorative care (Crown/Bridges, dentures) at 50%. (applies to in network providers
Vision Our vision plan is designed to provide for regular eye exams (once per year), frames (every 2 years), and lenses (each year) and contact lenses are covered at a discounted cost after co-pays and deductibles. (applies to in network providers)
Health Care Savings Account You may enroll for any amount from $260 to $2,500 for the year. You may use your Health Care Spending Account to pay your IRS allowed health care expenses Not covered by your health care coverage and those of your eligible dependents. There are many eligible expenses including: prescriptions, deductibles, co-insurance and co-payments, eyeglasses, contact lenses or LASIK and Dental and orthodontia expenses.
Dependant Care You may enroll in a Dependent Care FSA account for any amount from $260 to $4,550 for the year to pay expenses for a child or elder care for your eligible dependents. The care must be necessary to allow you and your spouse to work Or attend school full-time. Huber recognizes the financial demands of providing child and/or elder care and stretches your dollars even further by matching 10 cents to every dollar you contribute to this account up to a maximum annual Contribution of $450.
401K Savings Plan Huber encourages you to play an active role in planning your financial future by contributing to the Employee Savings Plan through convenient payroll deductions, and helps you share in the success of the Company at the same time through contributions made by Huber on your behalf. A wide range of investment funds are offered and investment changes can be made by phone or online. The 401(k) Savings Plan is administered by ING. Employee contributions of 1% to 75% of your pay: Huber match first 5% – dollar for dollar, above 5% – no match. Eligibility begins immediately and payroll deductions are made as soon as administratively possible following your enrollment.
Non Elective Employer Contribution Feature Following each Plan year, this feature provides for a tax-deferred Company annual contribution of 5% of your eligible pay. The Company’s contributions will be paid annually, provided you are employed for the 12-month period ending on December 31 of the eligible plan year.

Benefits (After Tax)


Life Insurance Basic, Supplemental, Spouse and Children The company provides basic life coverage of two times your annual base pay at no cost to you. You may elect additional life insurance coverage in increments of one to four times your annual salary, up to a maximum amount of $1,250,000 combined between the basic and supplemental coverage. You may purchase life insurance coverage for your legal spouse/civil union partner and child(ren) up to $100,000 for spouse and $15,000 for children.
Accidental Death and Dismemberment You may choose AD&D coverage for yourself only or for you and your family. Up to $300,000 combined for the family
Business Travel Accidental Insurance The company provides $100,000 of Basic Business Travel Accident Insurance (BTA) at no cost to you. You may also purchase supplemental coverage up to a $500,000 combined for basic and supplemental coverage. This coverage is limited to Huber business travel and does not cover your normal commute to work each day. Coverage is reduced beginning at age 70.
Long Term Disability Huber provides basic LTD coverage equal to 50% of your annual base pay at no cost to you. Supplemental: Basic benefit may be increased by an additional 10%.

No Cost Benefits


Short Term Disability You are eligible for this benefit the first of the month following 6 full months of continuous service. Huber will provide up to a maximum of twenty-six (26) weeks of benefits.
Survivor Benefit Huber offers a$2,500 survivor benefit paid to your designated beneficiary upon your death.
Employee Wellness Program There are many things you can do to help control your healthcare costs – using available preventive care programs, evaluating your health care choices and making sure any treatments are appropriate. The healthy choices you make today can lead to you and the Company spending less on healthcare tomorrow. All Huber employees that participate in the Huber medical program will need to satisfy the following three qualifiers to receive lower premium contributions: Be a tobacco-free family, complete an annual physical and/or biometric screening, and complete the Cigna Health assessment questionnaire, with numeric values.
HEW Wellness Program Going for the Gold quarterly fitness challenge offers monetary awards for meeting Bronze, Silver, and Gold level goals. An additional non-monetary prize is awarded for anyone who goes the extra mile and loses 10 pounds or more during a quarter.
Tuition Assistance Tuition Assistance is offered to full time employees after 12 month of service at 90% reimbursement.
HEW Profit Sharing HEW Profit Sharing Plan to all employees and is based on Eligible Pay.
Vacation and Floating Holiday Employees earn annually 2- 5 weeks of vacation based upon years of service. Also included are company recognized and floating holidays.
